Please fill out this form if you are interested in having your child included in the baby dedication ceremony scheduled specific Sundays during the year.
During the ceremony, you and your child will be asked to come up. Due to this ceremony including multiple familys, we ask that immediate family members only (Parents and sibling on dedicated child) come up for the ceremony. Please fill out the form as specific as possible so that we are ready for this special day.
